SPONSOR FORM

The idea of the Dance o'Clock event is to have fun and to raise money. Dancers must collect sponsorship for their chosen charity and need a personalised sponsor form that will attract interest. It must:

• be one side of A4
• include information about the dancer and their chosen charity
• indicate the time slots when this dancer will be taking part
• tell the sponsor about the event and how their money will help
• help the dancer to organise their sponsorship
• tell the dancer what to do once they have collected their money.

Create the sponsor form for your test buddy. Make sure it works when printed.

Make use of feedback from test users to ensure that the form is fit for purpose. This should include potential sponsors as well as your test buddy.
