Create a questionnaire. You must include the questions in this document.
You may add some questions of your own and choose the order of the questions.
You must design a suitable layout. Make sure that you include a sensible title and that the questionnaire is easy to complete.
Ask someone to proofread your questionnaire. You must then correct any errors.
Test your questionnaire by asking test users to complete it. Make any necessary changes.
Save your finished questionnaire. Make sure you use an acceptable file format.
